The post holder will be working in an integrated multi professional inpatient team and provide care under the supervision of a registered therapist such as a physiotherapist or occupational therapist, and the ward manager, undertaking rehabilitation interactions with a range of patients.

The post holder will provide quality care for a group of patients, specialising in rehabilitation and sub-acute care including both patients stepping down from acute hospitals and those stepping up from the community for acute hospital admission avoidance, in a physical health inpatient unit.

They will report to the senior therapist on duty or senior nurse on shift during weekends, and ultimately to the ward manager, enabling them to deliver services effectively and efficiently to all patients.

The post holder will assist in the care delivery of high quality care over a 7-day period that meets the needs of the patients.

The post holder may carry out will include undertaking assessments such as initial therapy assessments alongside providing rehabilitation supporting to achieve identified goals carrying out therapy group sessions, ordering equipment and other activities such as kitchen practice.

The post holder will be expected to communicate with patients who may have communication difficulties, families and other colleagues and document care provided on a computerised system.

Works within the multidisciplinary team under supervision of the therapy and nursing team.

Assists and carries out delegated duties in line with provision of quality evidence-based practice to patients, which includes following agreed protocols to assist in implementing and progressing set care plans

To work a variety of shift patterns which includes weekends and bank holidays on a rota basis.

Is responsible for implementing care and rehabilitation plans written by physiotherapists, occupational therapists and registered nurses.

Provides regular feedback to the registered nurse, physio and OT to allow them to review and adapt care/treatment plans accordingly.

Contributes to client goal planning sessions and show initiative in areas where competent and as agreed by senior staff.

The post holder will be involved in the induction of junior colleagues and provide support and guidance as necessary.
