We are seeking a dedicated and compassionate Rehabilitation Assistant to join our team at Active Care Group. As a Rehabilitation Assistant, you will play a vital role in supporting individuals with complex care needs to achieve their full potential.
Key Responsibilities- Provide high-quality care and support to individuals with complex care needs, promoting their independence and well-being.
- Work collaboratively with the multidisciplinary team to develop and implement personalized care plans.
- Assist with daily living activities, such as bathing, dressing, and grooming, ensuring the individual's dignity and comfort are maintained.
- Support individuals with their rehabilitation goals, including mobility, communication, and cognitive skills.
- Monitor and report any changes in the individual's condition, ensuring timely interventions and adjustments to care plans.
- Previous experience in a care setting, preferably in rehabilitation or complex care.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
- Ability to work flexibly, including evenings, weekends, and bank holidays.
- Willingness to undergo an Enhanced DBS check and maintain confidentiality at all times.
