Your role will involve intricate discharge planning for patients, working closely with therapists, social workers, and palliative care teams as necessary. You will also assist in patient rehabilitation, ensuring safe discharges and promoting independence, while working on a rotating shift basis.

Main Responsibilities:
- Assist the unit in overseeing individual patient care
- Guarantee the maintenance of a safe and effective service
- Provide person-centered care
Key Duties:
- Participate in patient care assessment, implementation, and evaluation
- Recognize changes in patient conditions and respond appropriately
- Ensure the delivery of safe, evidence-based care
- Effectively hand over responsibilities to the multi-disciplinary team
- Conduct tasks such as taking blood samples, recording observations, and administering medications
- Offer education and training to junior staff and engage in rehabilitation programs
