As a key member of the multidisciplinary team, you will be responsible for providing high-quality, individualized care to patients with neurological diagnoses, including stroke and brain injury.
Key Responsibilities
- Conduct comprehensive assessments and develop individualized treatment plans to promote patient independence and rehabilitation.
- Work closely with the multidisciplinary team to ensure seamless care coordination and effective communication.
- Provide education and support to patients and their families to promote self-management and empowerment.
- Collaborate with external health and social care professionals to ensure comprehensive care and support.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.

Requirements
- BSc Hons Degree in Occupational Therapy and valid HCPC registration.
- Minimum 12-18 months experience in neurology, with some experience in rehabilitation desirable.
- Strong communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Ability to complete comprehensive assessments and treatment plans, with a focus on patient-centered care.
- Car driver with full, clean driving license.
