**About the Role**:
The Primary Care Network (PCN) comprising of Ambrose Avenue Group Practice, Ardleigh Surgery and The Riverside Health Centre (ARA) is currently looking to recruit experienced Health and Wellbeing Coach to join our growing team.

Health and wellbeing coaches predominately use health coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. They may also provide access to self management education, peer support, and social prescribing.

We are currently looking for full time positions (37.5 hours per week) but will consider part time hours for the right applicants.

We are currently looking for someone who is passionate about working with patients and supporting the people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers. They may also provide access to self management education, peer support, and social prescribing.

This is a new post developed to provide non-clinical support for identified patients to help them to manage their health and well-being more confidently. These patients will be introduced into the service in a phased manner. You will also provide identified patients with an invitation to join the service and introductory information about personalised care planning, initial visits, guided conversations and assisted self-assessments using a Patient Activated Measure tool or other tool as decided by the service. You will support the patient in developing a personalised care plan, liaising with relatives, carers and other involved agencies as agreed with the individual to ensure an integrated and personalised approach to care and support.

**About Us**:
The Colte Partnership is a GP super-partnership, formed in June 2017. It brought together 9 local GP surgeries across Colchester and Tendring, serving approx. 98,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.

As well as General Practice, we offer Extended Access appointments to enable all of our patients to access medical care at evening and weekends; we work collaboratively with external agencies to provide in home services and Social Prescribing; we are working to be at the forefront of Clinical Research; and have many more exciting ventures on the horizon.

We also work as three collaborative Primary Care Networks (PCN’s) which span across Colchester and Tendring:

- Ambrose Avenue Group Practice, Riverside Medical Centre & Ardleigh Surgery
- Mersea Island Medical Practice, Tiptree Medical Centre & Rowhedge Medical Centre
- Walton Medical Centre, Colne Medical Centre & Wivenhoe Medical Centre

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support).
