**Context**

This post is to manage and coordinate the Short Breaks offer in Lambeth. This involves working with a range of stakeholders including families, service providers, schools, and social workers to ensure the smooth running of the service.

**Job Purpose**

1. As the coordinator, you will be responsible for managing incoming referrals, communicating with providers regarding availability, maintaining records of service usage, assisting with payments to providers, and overseeing the contract management of the service.

2. We are seeking a motivated individual who has a thorough understanding of short-break provisions for disabled children and can collaborate effectively with colleagues in the management of commissioned services.

3. In this role, you will be both a team player and a self-starter, able to develop projects and adapt to changes in legislation by creating literature for newsletters and designing spreadsheets.

**Responsibilities**

1. To coordinate the Short Breaks, offer in Lambeth, this includes managing referrals, working with providers to ensure maximum use of the Short Breaks services, speaking with parents regarding their referral, and how they can access the service.

2. In some circumstances, make decisions about which provision a child or young person should attend (in consultation with Social Workers where necessary).

3. Ensure that the provider’s submissions for payments are accurate ( reflecting the spot purchasing contract) and query where necessary.

4. To support with communications to stakeholders regarding the CWD service. This could include newsletters, online/Local Offer, and letters to families.

5. To keep records of who is accessing the services and ensuring that these records are accurate and kept up to date.

6. To support with the payments to providers, ensuring that these happen on time and with mínimal disruption.

7. Support with safeguarding alerts and help coordinate the information and any response required.

8. Work closely with the commissioning team to support the contract management of services and ensure that any required data are collected and made available.

9. Working closely with the finance team to manage payments and support budget management for these services.

10. Work closely with social workers to escalate any issues as necessary and provide information as required to enable social workers to support families.

**PERSON SPECIFICATION**

**Key Knowledge**

Understanding of short break provisions for disabled children and knowledge of childcare short breaks legislation.

Knowledge of children’s social care, specifically children with disabilities and SEND, is desirable.

Knowledge of national and local government policies, and challenges within local government.

**Relevant Experience**
- Experience within social care, health or education.
- Experience working with internal and external stakeholders and managing partnerships.
- Experience of working in a customer focussed environment.
- IT literate with excellent written and verbal communication skills.

**Other requirements**
- Hold a relevant professional or academic qualification.
- Enhanced DBS required
- 5-years references
