**Main duties of the job**:
You will be joining an innovative and dedicated multidisciplinary team and be responsible for supporting the MDT in the delivery of therapy intervention following assessment. You will be seeing clients within our ward facilities, as well as supporting the therapy team to assess clients in their homes or in the local community; supporting participation in a range of groups, carrying out treatment plans to support clients to achieve agreed goals. You will also support the team with relevant administrative support.

This position presents an excellent opportunity to further develop your clinical skills within a healthcare setting and will provide you with an excellent opportunity to develop skills within neurological rehabilitation. The team provides a supportive environment to develop within and offers peer support from other rehabilitation assistants. You will also be supervised and overseen by the highly skilled MDT.

We are looking for an enthusiastic individual with good general education to GCSE level (or equivalent), as well as previous experience of working with people in a health or social care setting. Applicants must demonstrate excellent communication skills and have the ability to work well in a team.

**Detailed job description and main responsibilities**:
To assist qualified therapists in the assessment and treatment of patients:

- To support the inpatient multidisciplinary team (MDT) to deliver rehabilitation and condition management service to patients with neurological conditions within the inpatient setting. The MDT includes: qualified physiotherapists, speech and language therapists, occupational therapists, neuropsychologists, vocational occupational therapist, dieticians, registered nurses and Doctors.
- To provide goal directed clinical interventions to patients with neurological disability who have been previously assessed by a member of the MDT
- Lead the delivery of therapist prescribed group exercise and education programmes
- To record and report back the treatment provided and the patient’s response to treatment in the relevant records in line with national and local standards.
- To undertake routine maintenance checks of clinical areas and equipment within designated clinical areas.
- To participate in 7 day working as required by site
- To observe, verbally report and document any progress, deterioration or problems to the prescribing qualified staff.
- To regularly carry out administration tasks to support patient care, activity recording, clinical governance and the multidisciplinary team process.
- To work flexibly to enable the provision of services that meet patient need.
- To work without direct supervision (as prescribed by the relevant qualified staff), working within their allocated role and adhere to the organisations procedures.
- To support patients to transition home; to include access visits to patients homes, supporting the qualified therapists to order, measure for and fit equipment and to meet with the technician at the patients home to ensure correct fitting of such equipment
- To carry out patient exercise programmes as prescribed by the therapists in both an inpatient rehab setting as well as at patients homes if required
