Due to the expansion of our Wellbeing services and schemes across Walsall and into Wolverhampton, we have an exciting opportunity for a Wellbeing Officer to join our Stronger Communities team. In this role you will be working with customers who are aged 55 and over, coordinating the delivery of a programme of health and wellbeing support to enable people to age well and live their best life possible.

**Main job responsibilities**:

- You will be the first point of contact for customers aged 55 and over who live within whg’s wellbeing schemes and with customers who live in the wider community, who have been identified as requiring support from the Wellbeing service.
- Using coaching conversations and motivational interviewing skills, you will hold Clever Conversations with customers, developing an environment which feels hopeful and happy and where everyone feels welcome and included.
- Working alongside colleagues within whg’s Community Housing Team, you will carry out accompanied viewings, ensuring the wellbeing scheme’s accommodation is suitable for the applicant’s housing needs.
- You will make sure that customers benefit from the full range of whg services and that you make referrals to others team as appropriate.
- Using a case management approach, you will use a pre agreed assessment framework to complete individual wellbeing assessments for customers. Using the results, you will co-produce an individual wellbeing plan which will seek to achieve outcomes set out within the PERMA framework as well as report on your performance outcomes to ensure they are captured in business wide KPI’s.
- Working alongside customers you will design an annual calendar of social events and activities which will reduce loneliness and isolation and maximise usage of communal rooms and other wellbeing facilities.

**We're looking for someone who has**:

- Experience of working with older and/or vulnerable adults using an asset strength-based approach
- Recent and relevant experience in social housing, health and adult social care, supported housing, community development, Social Prescribing or Mental Health services
- Awareness of the wider or social determinants of health and the potential impact upon an individual or wider community basis
- An awareness of current legislation which may impact upon our customers including a good understanding of Housing, The Care Act and The Mental Capacity Act
- Previous experience of using assessment frameworks, action planning for individuals and reporting upon achieved outcomes
- Ability to identify key risk factors affecting customers such as neglect, hoarding, loneliness and isolation and mental health issues.
- Good Microsoft Office skills (Outlook, Word and Excel)
