We have an exciting new opportunity for a
**Community Engagement & Assessment Worker** to act as a first point of contact to new to treatment and existing service users and providing Change Grow Live intervention as and when needed. You will be expected to complete comprehensive assessments of drug and alcohol users need and risk to individuals using Change Grow Live’s own case management system both in house and in the community linking in with external partners.

**Responsibilities**:
**About the role**:

- Provide triage and comprehensive assessments for service users looking to access treatment in both hub and spoke community venues
- Deliver Entry Into treatment POD groups as part of engagement
- Provide evidence based harm reduction and recovery orientated services
- Ensuring that all activities, and interventions are suitably recorded and in accordance with CGL policies, procedures and national guidelines from the PHE, DoH, NICE, NMC and as directed by the local management team
- Work with partnership organisations to develop and sustain effective comprehensive treatment pathways and broaden the client’s recovery plan accordingly
- To utilise a flexible approach and where required participate in activities and tasks which directly respond to client and service need

**About you**:

- Experience of working with substance misuse issues and a clear understanding of the need for and ability to deliver quality services
- Up to date knowledge of harm reduction strategies, and experience of promoting these messages to service users
- Knowledge of safeguarding children and vulnerable adult’s procedures
- Ability to work with and manage challenging behaviour
- Knowledge of local areas, geography and services (desirable but not essential)
