An amazing opportunity has arisen for an experienced and enthusiastic **Team Leader (Domiciliary Care)** to join **Complete Community Home Care Ltd** a rapidly expanding Care Organisation in Northumberland.

**The Role**:

- Build and maintain great relationships with service users and their families.
- Support our Care Managers to stay connected with service users and their families, undertaking Quality Assurance and Service Reviews, following up queries and working to ensure everything is perfect for our service users and their families.
- Meet prospective service users to develop outcome focussed care plans to help our service users get the most out of life.
- Be part of the Out of Hours On-Call team
- Be confident in talking to a range of other professionals and get to really understand and be able to explain your service user’s needs.
- Update care plans, audit activity logs and electronic MAR sheets alerting management to any issues or concerns.
- Maintain accurate service user and Care Professionals records on our electronic system.
- Always maintain confidentiality for all **Complete** **Community Home Care Ltd** Information, including service user, care professional & office data.
- You will be expected to do care calls as and when needed to support the needs of the business.

**Full-time**

**40hrs a week (Can vary depending on needs of the business)**

**Due to the nature of the role, Complete Community Home Care can only consider applicants aged 18 years and above, this is an occupational requirement and therefore is exempt from schedule 9 of the Equality Act 2010.**
