Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being Consult with patients presenting for acute minor illness assessment, treatment, and diagnosis at point of first contact within the ACT setting Assess, plan, implement, evaluate, and review individual treatment plans for patients with a known long-term condition Each individual will have a half day within their working week for their own speciality, to be decided with the individual at time of employment which will be beneficial to the needs of the patient population. This will be documented within the induction folder or professional development plan to be kept in your personnel folder Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition as appropriate Undertakes annual review of patients with chronic disease within the primary care setting and community as identified Undertake medication reviews in line with practice protocol as identified, face to face or remote as appropriate with patient Initiate tests and follow through results by assess to path links The Practitioner must be appropriately trained in radiation awareness and adhere to the Imaging referral protocol for nurse practitioners when referring patients for radiological tests Make professional autonomous decisions for which he/she is accountable Work with other health care professionals to diagnose, monitor, manage and treat conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols Work with patients in order to support adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions Work within the non-medical formulary in collaboration with GPs and other clinicians to meet the needs of patients Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote patients to live healthily and apply principles of self-care Support and manage health needs of women presenting for: emergency contraception /sexual health needs Implement and participate in vaccination and immunisation programmes for both adults and children Promote and deliver evidence-based care for patients presenting with aural conditions Meet the needs of patients presenting for wound management
