About the RoleThis Advanced Nurse Practitioner position is a unique opportunity to shape patient care pathways and contribute to innovative healthcare solutions in our dynamic primary care network. As a key member of our team, you will work closely with general practice teams, pharmacists, and other professionals to optimize patient outcomes.
- Main Duties:
- Participate in regular Care Home MDT meetings and ward rounds to assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients.
- Conduct clinical assessments for community-based patients and proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
- Support patients and Care Home staff to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
