Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate. Consult with patients presenting for minor illness assessment, treatment and diagnosis at point of first contact, by attending to patients in clinic. Undertake medication reviews in line with practice protocol as identified, face to face with patient.

Initiate tests and follow through results by assess to path links. Make professional autonomous decisions for which you are accountable. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Work within non medical extended formulary and supplementary prescribing in collaboration with GPs and other primary care nurse to meet the needs of patients.
