**About the role**

Working within the Information & Advice team as a Pathway Zero Coordinator you will provide professional and informed information and support to older people being discharged from Lincolnshire hospitals into community settings.

Providing telephone contact with each patient post discharge, you will check on a patient's wellbeing and provide signposting and referrals to community-based services; facilitating support to help them remain independent where appropriate.

The successful applicant will be based at our office on Park Street in Lincoln with a blend of home working.

The successful applicant will have a positive, enthusiastic and friendly nature.

An appreciation of the needs and an understanding of the issues affecting older people is required.

The ability to communicate effectively in both writing and verbally is essential to the role.

Excellent IT skills with a good working knowledge of Word, Excel and experience of using databases is expected.

Applicants are required to have a proactive approach, work independently and also effectively as part of a team to deliver an excellent service.
