An exciting opportunity for a dynamic and motivated advanced clinical practitioner to join First Coastal Primary Care Network has arisen. The ACP in Primary Care is a developing role nationwide and provides the opportunity to use advanced clinical skills to benefit patients. The successful applicant will act as clinical lead across the rural and coastal divisions and will take up a role that enables them to provide care at an advanced practice level and an opportunity to develop services that can meet the needs of the local population

We are looking for a highly motivated and autonomous person to be part of the First Coastal Primary Care Team. This service will provide holistic assessments, treatment and support to patients in their usual place of residence. You will be part of a service in which care is provided by a multi-skilled team including nurses, mental health practitioners, social care, occupational therapists, clinical pharmacists, dieticians and care coordinators.

With a proactive approach to health care, the team visits patients in their own homes, care homes and clinic providing holistic assessment, long-term condition management and onward referral, alongside personalised care and support planning. Patients are aged over 18 and are likely to present with frailty and falls or deterioration of the condition.

The successful applicant will provide clinical leadership, education and support to others and will be involved in the development of services across the PCN footprint. They will be responsible for implementing, monitoring and the continued development of the Enhanced Health in Care Homes initiative.

- To act as clinical lead for the rural division, demonstrating effective clinical leadership and management.
- To provide complex assessment, treatment, support and goal setting for patients.
- To ensure a high standard of care for all patients by effective communication with patients, carers, GPs and other health professionals.
- To work as an autonomous practitioner, undertaking specialist assessment of those who have complex needs using analytical and investigative approaches
- Ensure that the team has the appropriate levels of knowledge and skills, provide specialist advice to more junior colleagues in the management of the caseload.
- Work effectively with others to develop services and create integration with other organisations.
