**Who are One Fylde?**

At One Fylde we are passionate about offering caring support, safe homes and meaningful activities to adults and young people with learning disabilities across the Fylde and Wyre.

Our Outreach Team supports over 60 people with learning disabilities across Lytham St.Annes, Bispham, Thornton, Fleetwood and over Wyre.

**What is an LD Outreach Worker?**

We like to view our Outreach Workers more as "life coaches" being able to provide guidance to the people we support as well as have the tools and knowledge of handling bills, supporting to increase one's self esteem, be an agony aunt, a chef when helping with cooking and a tour guide when travelling abroad Our focus in Outreach is to support people to live as independently as possible, to maintain their home environment, and manage their health, finances, social activities, hobbies and interests, this could be anything from supporting someone to manage their weekly shop, visit friends and family or even to go on Holiday.

The term "Outreach Worker" does not cover what we as One Fylde do for the people we support. If you feel like you have the passion and tools to let us take you under our wing then please do apply.

Not every day is the same on Outreach, you have to be adaptable to the people you are supporting and punctual with your time management. Most of our support revolves around supporting our individuals to access the community whether this is supporting someone swimming, to the cinema, golf or slimming world.

For those of you who have not done support work before, full training will be given, in fact we offer full and wide ranging training opportunities for all, in a charity that encourages personal development.

Key responsibilities:

- Maintaining a quality service
- Ensuring that the people we support have a safe, appropriate, and comfortable living environment.
- Communicating effectively with both internal and external stakeholders, including families and social services.
- Following support plans and other relevant information regarding how to assist the people we support, including Health and Safety requirements.

There is a variation of different shift patterns available within this role, most shifts being day time visits on various days Monday - Sunday. We have part time and full time positions available, also weekend only working patterns too.
