**About the role**:
This is an exciting opportunity to work as a Community Support Officer in the Occupational Therapy Team in North Powys. We are a friendly and supportive team. Your role will be to assess people’s needs and provide advice, equipment, and adaptations in people’s homes to support them to live independently and safely. The post is fixed term until March 2025 to cover maternity leave.

**About you**:
Good knowledge of a range of medical conditions and disabilities and the impact that on a person’s functional ability and ability to manage everyday occupations. A passion for working with people and enabling them to achieve their goals and live in an accessible home environment.

**What you will do**:

- Gather further information on people’s needs.
- Assess people’s needs under Social Services and Well Being Act and Housing, Grants, Construction and Regeneration Act 1996.
- Record information accurately on the council’s ICT systems,
- Make appropriate referrals to partner agencies,
- Work with social work, housing and other teams and agencies to support a person to meet their outcomes.

**Job Types**: Full-time, Fixed term contract

**Salary**: £27,803.00-£29,777.00 per year
