This is an exciting opportunity to work across in

LDA Hospital at Home is an innovative new service being developed under the Thames Valley Provider Collaborative. The Hospital at Home team will focus on young people with moderate to severe learning disabilities AND/OR a severe degree of functional impairment associated with an autism diagnosis who are at risk of home or placement breakdown and who could meet the threshold for inpatient care.

The in-patient units, Highfield and Meadow both offer support and therapeutic containment for young people in a safe and secure environment.

The Highfield is a specialist in
- patient service for 11-18 year olds with acute mental health needs. The unit offers a safe and supportive environment while young people are in the most vulnerable stage of their illness.

The Meadow unit is the new PICU, is for 11-18 year olds and offers increased intensity and care for the young people who are requiring hospital admission.

As our Speech and Language therapist, you will ensure the systematic provision of a high-quality specialist speech and language service to clients under the care of the LDA Hospital at Home team, the Highfield team and the Meadow Unit.

The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible. Being part of the Thames Valley Collaborative offers several opportunities for staff development and training. You will be based in Oxford in our new offices located above the meadow unit on the Warneford site.

This is an exciting time to be part of the development of a new Tier 4 service for CAMHS and to be part of the hospital services within CAMHS. The teams all have close links and working relationships across the Thames valley collaborative area to support existing provision across the patch. The main functions of all the teams are to ensure that the right level of care and support is offered to the young people in the area in a patient and family centred model of care.

The Provider Collaborative will ensure that community and inpatient provision continue to work as an integrated pathway ensuring children and young people’s care is seamless, timely and appropriate to meet needs. This multiagency approach through the Provider Collaborative supports developments to provide alternatives to admission and admission prevention.
