Clinical Operational Lead - Gloucestershire Virtual Wards Medical Hub, Band 8a

Are you looking to be part of something new and exciting in Gloucestershire? If so, we have a fantastic opportunity for you to lead and influence the development of our fully integrated Virtual Ward. We are recruiting a team of experienced and enthusiastic clinicians to establish a Virtual Ward Hub for Gloucestershire and provide the link between our community teams and acute hospital pathways.

We are recruiting an experienced Clinical Operational Lead to work alongside the Lead Consultant and community leads to develop the service; enhancing partnership working across secondary care, community, and primary care teams. Applicants from a wide range of backgrounds are welcome and we hope to recruit a truly cross system team
- bringing a range of experience and expertise. The postholder will be expected to take part in a rota providing cover to the Virtual Wards from 8am - 8pm seven days per week. Initially the hub will be located at the acute hospital site and the team will work alongside the medical SDEC staff, there will be an expectation of flexibility of base across community and secondary care as the service develops.
- Liaise with the relevant senior medical, nursing, allied health professionals (AHPs) and pharmacy teams and stakeholders across the Integrated Care System (ICS)
- To co-ordinate and lead in the planning, development, management and provision of a medicine virtual ward hub
- To plan, develop and deliver appropriate training and education to healthcare colleagues to promote the safe referral to and management of patients on the virtual ward.
- Take a lead role in identifying, developing, and supporting policies and procedures for referral and management for patients on the virtual wards. Ensuring the operational safe operational delivery of care and cover across the hub 7 days per week.
- To work closely with our digital tech provider to ensure that patient alerts are dealt with in a timely and appropriate manner with escalation as required.

The Virtual Ward programme is a truly whole system initiative.

This role will be employed by the acute trust with an honorary contract with our community provider - we believe that this role is crucial to the interface between community and secondary care, and this role will build on the great partnership working already established between the 2 providers for the delivery of excellent care. Our digital partner Doccla has a wide range of Virtual Ward experience from other systems - and we are leading the way with two way integration into our electronic patient records.
