In this role you will be ensuring the most appropriate care is provided to all patients.

In this role you will be expected to use your skills to care for patient’s in the Community and Care Home setting.

We work collaboratively with members of the multidisciplinary teams aiming to ensure appropriate treatment is available to all patients in their home, provided out of hours.
- This job description is indicative only and the role will be reviewed at least annually as part of the appraisal process to take account of changing needs /development of the service
- To meet the evolving needs of the organisation you may also be required to provide cover in other areas following appropriate discussion
- Work from office base but visit patient’s in their own homes to provide care following the patient’s individualised care plan.

- To work as a Healthcare Support worker to support the multidisciplinary team, in the effective and efficient provision of care to aid patient’s recovery, promote independence, health and wellbeing following illness or deterioration.
- Provide thorough individualised care plans as set by nurses, paramedics and therapists.
- Work as a member of the multidisciplinary team observing, recording and reporting any changes in a patient’s condition or circumstances.
- Support patients in the community, providing care and support when there is a patient need.
- Be familiar with the steps necessary in an emergency and be able to act accordingly.
- To provide planned, delegated therapy and nursing interventions for patients in their own homes or other settings as appropriate.
- To demonstrate independent working without direct supervision.
- To participate in shared care with other agencies / services as required.
- Provide accurate feedback on patient’s care to the Nursing teams.
- Ensure the needs of carers and relatives are listened to and reported back to the Nurse in Charge of the shift.
- Ensure clear, concise and accurate records are kept and updated in a timely manner, including the use of departmental electronic records.
- To be aware of health and safety and follow risk assessments to ensure the safety of service users and staff.
- To undertake relevant training as provided.
