**Job overview**:
We have an exciting opportunity for 2 Therapy Support Workers, 22.5 hours per week. One based at Camborne Redruth Community Hospital and one based at Liskeard and/or Bodmin area to join our Community Stroke and Neurology Rehabilitation Service. The service provides specialist assessment and rehabilitation for patients with a wide variety of acquired and progressive neurological conditions, including Stroke. This is a very friendly, enthusiastic, and dynamic team. You will be working with experienced occupational therapists and physiotherapists as part of an integrated multidisciplinary team who will give support, joint working, regular supervision, appraisal, in-service training and a real commitment to professional development.

**Main duties of the job**:
- To manage and prioritise own patient caseload including management of physical and functional rehabilitation of patients as delegated by the therapist.
- Support the therapists with joint sessions individually or in a group in a variety of settings including clinic, community and home environments.
- Deliver therapy intervention within the Stroke and Neurological Service within competency and capacity required.
- Communicate effectively with patients, relatives, carers, other services and the Stroke and Neurology Rehabilitation therapists and manager. This will include attending meetings, highlighting any problems and providing regular feedback of patient progress and any issues within service delivery.
- To make accurate records of work undertaken and keep statistics as required, including documenting patient contacts in line with COT, CSP and service standards.
- To keep up to date on professional issues by participating in In-Service Training and other training activities as agreed through the appraisal process.
