**What you’ll be responsible for**:

- To work clinically with service users in order to help prepare them for discharge from hospital. Working alongside the Pathways Team Leader, the inpatient wards, CRHTT, CMHT’s and other agencies to support and plan clients timely discharge from hospital.
- To support the planning, coordination, and transfer of service users across care pathways, liaising with Southern Health units and other NHS or private providers as required maximising the efficient use of inpatient services and transferring to other units as the client’s presentation necessitates.
- Alongside the Team Leader to provide support, guidance, and supervision to a small team of Band 4 Discharge Facilitators who work alongside Pathway Co-Ordinator to undertake administrative tasks to efficiently monitor clients discharge planning.
- To assist and support the development, provision and review of care plans in cooperation with nominated clinicians.
- To undertake assessment of service users in relation to their needs and contribute to how needs can best be met. To plan, implement and review the effectiveness of therapeutic interventions as required within care pathways and placements.
- To liaise with carers and review support for carers and families; working collaboratively when consent is agreed.
- To facilitate service user access to services including - planning and coordinating follow up care and signpost and coordinate links with other care providers as appropriate.
- Plan and implement discharge and transfer pathways between SHFT services and other specialist services.
- To develop and implement strategies which promote the value of people as individuals to advocate and enable service user’s and carer’s rights to be promoted.
- To work as a role model in placing the service user’s experience at the core of service delivery.
- To accurately record and update a database of clients under the care of the transfer facilitators in their nominated area, providing relevant information in a concise, timely manner providing relevant information to local and senior managers.

The role will involve, but not be limited to you performing the above.

- First Level Registered Nurse
- Clinical supervision training
- Computer skills
- Experience post qualifying
- Mentorship course
- Ability to travel
- Knowledge of another language
- Speciality specific Level 3 learning

**Knowledge and Skills**:

- Highly developed communication skills
- Ability to motivate service users
- Ability to communicate effectively with people who have difficulty understanding
- Good facilitation skills
- Evidence of effective interpersonal skills (active listening, building empathy)
- Ability to organise self and others to achieve tasks
- Able to co-facilitate group work
- Evidence of experience in education of nurses
- Evidence of effective team working and leadership skills
