As Clinical Support Worker for Virtual Hub you will be responsible for providing a range of both clinical and administrative duties under the guidance of the team leaders in onboarding & supporting patients in the provision of remote monitoring & management of acute care in the patients' place of residence.

This is a permanent full time post (37.5 hours) working across Monday to Sunday.

As a support to the Virtual Hub, the Clinical Support Worker will be required to work independently performing several clinical duties/tasks following training and meeting competency within role as per policy and procedure.

An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks.

This post involves contact with patients and families face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

- Support with the organisation of remote monitoring equipment delivery and equipment, working across the trust localities and performing home visits.

- Be aware of the patient’s needs - emotional, social, physical, and cultural. Respect privacy, dignity and beliefs. Acts in a way that supports equality and diversity values.

- Report any information regarding the wellbeing of the patients to the Registered Health Care Professional in a timely manner.
- To Assist the Virtual Hub Administrator with patient flow on clinical systems.
- At the request of the Registered Health Care Professional, perform and record physiological measurements within the remit of the Clinical Support Worker role and report accurately to the Registered Health Care Professional, the assessment undertaken, on patients.
- Assist the Virtual Ward Team in maintaining a clean environment
- Provide and receive sensitive information concerning a patient’s medical condition.
- Work with the registered heath care professional and engage with other agencies and community teams to ensure a safe, individualised approach to the admission and discharge of patients.
- Actively participate in and attend ward/unit meetings.
- Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
