**Band 6 Mental Health Practitioner Advert**

**We are looking for a creative and enthusiastic Band 6 Mental Health Practitioner who is passionate about addressing the healthcare inequalities within central Bradford to join the Proactive Care Team.**

**The Proactive Care Team is a relatively new team that aims to develop and deliver a community based integrated model of healthcare for individuals with the most complex health needs, within the three Community Partnership areas of the Central Locality in Bradford. The Proactive Care Team works to help reduce health inequalities for those with the most complex needs and support a reduction in Accident and Emergency Department attendances and unplanned admissions, whilst also improving access to other existing community-based provision to help sustain good health outcomes. Team members deliver a blend of preventative and recovery focussed interventions in the community, to support a reduction of premature mortality and help deliver improvements for those with complex physical and mental health needs, those who are frail and those close to their end of life.**

**We have an exciting opportunity for a Band 6 Mental Health Practitioner to be part of our service, developing the mental health provision within the Team. The role will involve working closely with the wider MDT, to provide holistic assessment and appropriate intervention for service users referred to the Proactive Care Team, from diverse communities, who have mental health needs.
