An exciting opportunity has arisen within the Paediatric Ophthalmology Team for two Trainee Advanced Clinical Practitioners.

PLEASE NOTE THIS POSITION IS FOR INTERNAL APPLICANTS ONLY

The postholders will be key senior members of the team, providing paediatric eye care in Sheffield and across the region. We are a fantastic team, who drive forward service improvements, are highly family centred in their approaches and are a fantastic collaborative team, who care for and look after each other.

You may be required to see patients on wards or A&E and be able to manage the complete pathway for specific conditions.

The post may be worked at any Trust location.

As an existing member of the Ophthalmology team you will have experience and interest in paediatric assessment and treatment and be committed to child / family centred approaches.

The post holder will complete a portfolio of competency and evidence based practice, and will work towards fulfilling the core capabilities of an ACP as set out in the Trust and HEE framework for advanced clinical practice in England (HEE, 2017), and the qualified ACP job description.

This post is at Band 6 or 7 depending on current position for the first year of training, rising to Band 7 for year 2 and 3. It is expected that this will lead to a qualified role as an ACP at Band 8a after a further 6 month consolidation period post qualification. The role is open to flexible working/part time if required for a minimum of 30 hours across 4 days per week during training and when qualified.

The position offers the opportunity to develop a strong leadership role within the department. The post holder will develop skills to act as a role model, developing their knowledge and skills to work towards delivering an advanced level of practice in the speciality whilst delivering evidence-based high-quality care for children and their families. This will include developing the skills to become an autonomous practitioner including undertaking clinical assessments, formulating a differential diagnosis, ordering and interpreting clinical investigations, using medical exemptions or prescribing if applicable, clinical decision making and planning of care and treatment. The post holder will work closely with all members of the multi-disciplinary team playing an integral role in the operational and strategic development of the advanced practice.

Experience and skills will be underpinned by the four pillars of advanced practice: clinical practice, leadership and management, education and research.

You will be involved in audit and research to enhance the service and also present at national/international level and support a learning culture within the department.

Clinical practice

**Under supervision the post holder will**:
Develop and utilise expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate evidence based clinical management of patients within a defined scope of practice. This may include performing refraction and fundus examinations where indicated.

Manage complex episodes of care, and follow up working in partnership with all members of the multi-disciplinary and multi-agency teams.

Provide support and advice to patients, parents and colleagues contacting the department concerning abnormal ocular conditions

Work closely with all members of the multi-disciplinary team, including administrative, nursing, medical and orthoptic disciplines to provide an integrated service.

Be aware of and involved in current trends in paediatric ophthalmology and introduce new ideas to promote high standards of evidence based patient care.Develop collaborative links, locally and nationally.

Leadership, training and mentoring

Work in collaboration with the team to plan and deliver interventions to meet the learning and development needs of the wider team and in particular junior colleagues and students across all professional groups.
