This is an exciting time to join us in the Adult Community Services Directorate as we embark on delivering the ambitions described in the NHS Long Term plan. Implementation of our ‘Home First’ model is under way and the successful applicant will be part of the journey in the development of Virtual wards.

Virtual Wards allow patients to be cared for at home and not have to be admitted to hospital. This is part of a national agenda to improve patients experience and avoid admission to hospital if clinically safe to do so.

As the Administrator for Virtual wards, you will work in partnership with Oxleas and Lewisham and Greenwich Hospital staff, and both internal and external agencies to ensure that processes are supported with a high standard of administration. The post holder will proactively contribute to the evaluation of data and support clinicians with the day to day running of the Virtual Ward.

The post would be suitable for an experienced administrator with knowledge of clinical systems including RiO and ICare.

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. Our multidisciplinary teams look after people of all ages and we work in close partnership with other parts of the NHS, local councils and the voluntary sector and through our new provider collaboratives. Our 4,300 members of staff work in many different settings including hospitals, clinics, prisons, secure hospitals, children’s centres, schools and people’s homes.

We have over 125 sites in a variety of locations in the South of England. In London we operate within the Boroughs of Bexley, Bromley Greenwich and into Kent. We manage hospital sites including Queen Mary’s Hospital, Sidcup and Memorial Hospital, Woolwich, as well as the Bracton Centre, our medium secure unit for people with mental health needs. We are the largest NHS provider of prison health services providing healthcare to prisons within Devon, Dorset, Bristol, Wiltshire and Gloucestershire, Kent and South London. - Recording and monitoring the outcomes of regular meetings including the performance improvement action plans with agreed partners.
- Documentation of the virtual MDT meetings, ensuring attendance and engagement of key players
- Developing system/data base of this cohort of patients.
- To be responsible for the development of the administration function and management of office systems
- To maintain and update service activity information on both electronic and manual systems
- To enter computer data in support of the development of clinical information systems within RIO, including uploading documents on RIO.
- Ensure all referrals are registered, scanned and uploaded on RIO daily
- To ensure that all patient data is distributed to clinicians as required in a timely manner
- To act as first point of contact for the service, taking the initiative to deal with enquiries in a proactive manner.
- To be an advanced user of IT packages including Word, Excel, Power Point as required for complexity of work.
- Handling both routine and urgent matters using initiative and with mínimal supervision.
- Developing and maintain efficient office systems.
- To provide support to the team and develop databases to log patient activity.
- Support the service in the provision of ad hoc information requests.
- Will be required to meet deadlines and supply information at short notice
- To obtain daily updates from patients as directed by clinical staff.
