Mental Health Link Practitioner

SBUHB Primary Care and Community Services, working alongside Mental Health and Learning Disabilities Delivery Unit are pleased to present an exciting opportunity for a part time professional to join the Local Primary Mental Health Support Service across Swansea in a developing role.
Each Mental Health Link Practitioner are aligned to specific GP clusters and will aim to interface, liaise and educate our primary care colleagues about our service in line with Part 1 of the Mental Health (Wales) Measure 2010. The role will involve working closely with the GP’s in each cluster and where needed provide education and training when identified or need expressed on LPMHSS. The role will also include the practitioner having brief conversations with patients to determine whether a full mental health assessment is needed, if this progresses during the contact, then the link practitioner can follow this through to a full assessment. They will help plan, develop and evaluate primary care mental health services in that area and negotiate care provision across services, working closely with voluntary organisations. They will be an additional resource to the existing Assessor Practitioners and will be involved in the screening and assessment process of individuals within primary care. In order to fulfil targets set by WAG, link practitioners will be expected to assist in carrying out full mental health assessments alongside Assessor Practitioners in the team.
The post is open to Registered professionals with a qualification in mental health nursing, occupational therapy, social work or clinical psychology.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
