This is a joint funded Ofsted registered children’s home operated through an integrated health and social care staffing model to care for Children Looked After.

The home will provide up to 4 beds (3 short stay, one emergency in a separate annex) for children with mental health presentations who are repeat attenders at A&E /S136 suite or at risk of /or have had admission to Tier 4 provision. The service will also admit children as a step down from Tier 4 provision.

JOB SUMMARY

New Farm House is an Ofsted registered children’s home operated through an integrated health and social care staffing model (HPFT and HCC Social Services) to care for children looked after. The home provides 4 beds for children with mental health presentations who may be repeat attenders at A&E /S136 suite or at risk of /or have had admission to Tier 4 provision (psychiatric inpatient unit).

The home uses the ARC theoretical framework (Attachment, Regulation and Competency) and Non-violent resistance interventions and ethos in the provision to create a warm and nurturing environment to enhance wellbeing of the CYP. Training is provided and all workers are expected to work within this framework. The post-holder will work within professional guidelines and under the clinical and professional supervision of a Clinical Lead alongside the Registered Manager.

The post holder will deliver a high quality service working with service users and staff to ensure practices that respond effectively to both individual and service need. The role will provide effective clinical leadership to ensure all nursing staff receive the appropriate supervision and support in conjunction with an agreed professional development plan. The post holder will adhere to Ofsted regulations and take the lead in the absence of the Registered Manager.

Furthermore, the post holder will be responsible for establishing and maintaining team systems and processes for effective operation of the team, including outcome measurements, audit and annual service policy review.

Clinical Responsibilities
- To ensure effective nursing assessment and provision of a high standard of care within the
- To provide mental health expertise, clinical support and direction for complex or high risk cases and support with escalating concerns with cases through the appropriate routes.
- To take an active role in implementing preventative measures and clinical de-escalation and management for children within the home to prevent crisis and AE attendance.
- To provide a safe and containing environment that will help stabilise the mental health of children experiencing a mental health crisis in order to allow them to return to their original placement and thus enhance placement stability
- To act as a positive clinical lead to ensure there is shared risk management and support in the management of children and young people in the New Farm.
