Cancer Services Directorate provides comprehensive pathway coordination and tracking function for patients with known and suspected cancer.
We are responsible for all submissions to Cancer Waiting times, The Cancer Registry, Peer Review and National cancer audits and Datasets. Confidence in operating IT systems is essential.
An opportunity has arisen for a substantive Cancer Pathway Tracker to work within the Cancer Performance Team to undertake the tracking of patients through their cancer pathway.
Your work will be vital in supporting the timely and quality care for patients with suspected or confirmed cancer and in ensuring that the Trust continues to provide a high quality of service for cancer patients..
We have 3 positions available ;
1x Substantive
1x 12 months fixed term - Maternity cover
1x Part time job share - 18.75 hrs per week
- To coordinate successful monitoring of patients on the Cancer Patient Tracking List (PTL), helping to ensure that a patient is diagnosed and treated with in their Cancer Waiting Times date, while helping to ensure a positive patient experience.
- To assist with multi-disciplinary team (MDT) meetings in order to achieve the national Cancer Waiting Times (CWT) standards. To collect Cancer Outcomes and Services Dataset (COSD) information for patients with cancer diagnoses and to collect national cancer audit data.
- To ensure that PTL meetings run effectively and are well structured. To ensure patients are discussed according to evidence-based protocols and that the MDT meetings meet the requirements of the Trust’s MDT SOP.

Cancer Data Tracking
- Monitor patient progress along the Cancer Waiting Times treatment pathway by:
- Monitoring the Trust PAS, diagnostic systems and MDT outcomes for details of patient activity
- Supporting the MDT Coordinators in managing the Trust’s cancer patient tracking list
- Managing patient progress against the Trust Cancer Escalation Policy
- Support the collection of clinical staging data and the wider Cancer Outcomes and Services Dataset
- Support the collection of data for the various national cancer audits, including LUCADA, NBOCAP, DAHNO
- Support the Deputy Cancer Performance Manager with ad hoc data collection
- Any other data information requested within the scope of the role.
- Speaking with patients in a polite and professional manner while helping to organise outpatient appointments, scans, etc.
- Ensure that actions from the weekly PTL are escalated appropriately and in a timely manner.
Service Management - Regular Reports
- Assist with the preparation and implementation of new, regular reports as required.
- Adapt and improve information systems and reports within own area.
- Communicate analytical / statistical findings to non-analytical professionals.
- Support the population of data in the Trust’s cancer information management system as directed by the MDT Coordinators and Corporate Cancer Management team to meet the requirements of the reports outlined above.
- Through daily tracking, identify trends and pathway delays and escalate to the appropriate manager.
Service Management - Adhoc Reports
- Contribute to reports from the various systems used within Cancer Services in response to ad-hoc requests from internal and external organisations.
- Work with the CWT Manager to provide correct patient tracking information to support escalation to operational services thereby improving performance and enhancing patient experience.
- Assess whether analyses are robust, investigate data anomalies identified during analyses and instigate corrective action where required.
- Communicate analytical / statistical findings to non-analytical professionals.
- Adjust work plan accordingly to deal with urgent requirements e.g. prioritisation of daily tasks.
