Birmingham, United Kingdom Sandwell and West Birmingham NHS Trust Full time

An exciting opportunity has arisen to join Sandwell and West Birmingham Hospitals as a Band 3 Patient Flow Assistant within Acute Medicine.

The role will be based cross site at Sandwell General Hospital and City Hospital, with rotation across the two sites expected. During Spring 2024, the organisation will move to the new Midland Metropolitan University Hospital in Smethwick, which will transition the team to single site working.

**Shift Patterns are as follows**:

- Long Days 07:30 - 19:30
- Monday to Sunday
- 34.5 Hours per Week - 3 Long Days
- 23 Hours per Week - 2 Long Days

The purpose of the Patient Flow Assistant (PFA) role is to ensure that patients have a planned timely transfer or discharge, and that the patient's pathway is managed, ensuring all patients have a positive experience of the hospital admission or discharge process.

The PFA will be based on the Acute Medicine Unit supporting the Clinical Shift Lead/Nurse in Charge/Unit Lead to ensure that transfer or discharge planning is pro-active and proceeds without unnecessary delay.

It is also expected that the PFA will work closely with the Medical, Nursing and Support Service Staff to ensure that patients are proactively managed through the system, checking that elements of the patients pathway such as investigations, medications and transport are being requested in a timely way.

Sandwell and West Birmingham NHS Trust is a high performing Trust achieving a 'good’ rating in our most recent Care Quality Commission (CQC) assessment and achieving significant progress in the delivery of our strategic objectives. We are proud of the commitment of our staff to ensuring a strong and sustainable future for this organisation which is highly valued by our local communities.

There has never been a better time to join SWBH. It is moving at pace and a ‘we can do this’ attitude is evident right through the organisation, where staff are engaged for change and committed to being part of the journey to sustainability and growth moving into the new Midland Met Hospital in 2024.

We take equal opportunities and improving the working lives of our staff really seriously, this could include flexible working, part-time, job share and remote working from home. Please talk to us during the interview process to discuss any flexibility that you may require.

Please see attached Job Description and Person Specification for full details of what this role entails.
