Job Description Job purpose Under the direction of a Team Manager and with support and direction from Social Worker colleagues, provide a high quality and comprehensive assessment and support planning service, advising adults with care and support needs. In doing so, promote the principles of wellbeing as described within the Care Act 2014. Responsibilities To work within a community or hospital based team, providing person centred support in line with the Care Act 2014. Contribute to the promotion of improved outcomes for individuals, families, carers and groups from diverse and disadvantaged communities.

To undertake and contribute to the review of individual plans relating to the assessed needs of service users, their families and carers. Maintain and update case notes and other electronic records, write reports as required within the limits of data protection. With supervision, to carry a caseload of appropriately allocated cases and undertake assessments of service user needs using a person centred outcomes focused approach. Where required, prepare person centred support plans in conjunction with service user, their families and carers.

Where safeguarding issues arise on the Wellbeing and Recovery Worker caseload, to escalate them to the manager and to assist the Social Worker to investigate and manage risks. To ensure that any concerns about practice and procedures are raised with the team manager. Organise work activities on their caseload and prioritise tasks as required. With support to undertake office duty as required.

Be responsive to feedback and self-reflection around practise. Appropriately participate in meetings and relevant forums. Work closely with line manager using supervision and appraisal to develop own performance and skills through CPD. To use supervision and appraisal to continually develop professionally.

To comply with the requirements of all Leeds City Council and Directorate policies and procedures and staff instructions, including responsibilities under Health and Safety policy and procedures. To actively promote Equality and Diversity throughout the service. To undertake any other duties and responsibilities appropriate to the grade and purpose of the job as may be determined by the directorate. Ability to plan, prioritise and time manage own workload. Able to communicate well and with a range of partners and service users, carers and colleagues.Able to engage individuals, families, carers and workers from other agencies.Ability to devise and implement service and support plans. Ability to assess the needs of and work with individuals and groups for people using a person centred approach. Ability to maintain accurate service user records using IT and follow policy and procedure.

Able to exercise individual judgement, assess risk and consult where and when appropriate. Report writing and Communications Skills. Knowledge of relevant legislation appropriate to the needs of service users from a wide range of backgrounds. Awareness of the broad range of factors that impact on service users and families e.g.

Demonstrate an awareness of needs and safeguarding issues associated with vulnerable people. Demonstrate an understanding of diversity issues within local communities. Experience of working with a range of people who may have care and support needs. Demonstrated creativity in tackling and solving problems.

Experience of advocacy and user involvement. Experience of using initiative and a commitment to developing services. Experience of producing written records and reports to a high standard for a variety of purposes with language suited to function. Application of information gathering skills to make and contribute to assessments.

Awareness of the use of advocacy and user involvement in a Social Care setting. Experience of implementing a person centred approach.
