**About Islington N1 Primary Care Network and Background**

Islington North 1 Primary Care Network (PCN) consists of 4 practices and our new DTL will be an integral part of the Senior Management team of the PCN.

The population is diverse, often with significant and complex health and wellbeing needs. There is an exciting opportunity for an enthusiastic and motivated individual to support the co-ordination of the care provided and work closely with people to improved health outcomes and a better quality of life.We are looking for a data driven expert for an exciting new role within the. We want this expert to perform a data driven review of how practices work across the PCN to inform changes that positively impact patient care and internal staff, as well as improve engagement and access specifically around the new LCS contract. This is a six month contract, with potential for extension.

We want someone who is curious and interested in technology and has an understanding about integrating it into existing systems. The post-holder is expected to have an understanding of the challenges in Primary Care and knowledge of the technology that supports it.

The post holder will work closely with the Clinical Director, Operational Manager, Practice Managers and other staff within the PCN.. The post holder will need to delivery collaborative changes to support service improvements, identify opportunities and make tangible positive transformation.

As this is a new role and the guidance around PCNs is constantly changing, there is a need for the postholder to be flexible and adaptable in this evolving environment.

**This role will include delivery of a combination of the following responsibilities**:

- Improve adoption and/or optimisation of existing technology to enhance patient access and experience whilst increasing PCN productivity.
- Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access).
- Review, improve and support the Practices digital maturity
- Use quality data to show value and best practice across the PCN

**Understand demand, capacity and activity to drive improvements in**:

- Patient experience of access**.**
- Operational efficiency, including better matching of capacity with need.
- Staff experience at work through reduction of workload and increasing a sense of involvement in the care and wellbeing of patients.
- Experience of working in general practice or with a known supplier (eg EMIS/Apex Edenbridge) with a good working knowledge of general practice systems (including IT, digital and BI tools), processes and procedures.
- Deep understanding of ACCURX, EMIS and other stratifying tools (eg Healthintent, Apex Edenbridge)
- Ability to extract, collate and interpret data to drive positive change.
- Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers available to support the Transformation Team.
- Ability to interpret qualitative and quantitative information from a range of sources
- Excellent communication skills
- An ongoing commitment to improving NHS services for the benefit of patients.
- Have a flexible, “can do” approach to work with a passion for delivery.

**Person Specification**

**Qualifications - Desirable**
- Degree or equivalent qualification or relevant experience in primary care

**Experience - Essential**
- Experience delivering excellent customer service and understand the importance of patient-centred care
- Experience of using EMIS and other tools used in primary care

**Skills & Attributes - Essential**
- Fluent with Information Technology and proficient in MS Office
- Strong analytical thinking and ability to handle multiple tasks concurrently and prioritise tasks
- Can deliver technical information in a non-technical way to a range of stakeholders
- Excellent numeracy and data interpretation and reporting capability
- Can communicate effectively in a range of situations, from presentations to 1:1
- Ability to work on own initiative and organise workload, allocating work as necessary
- Adaptability, flexibility and ability to cope with change
- Excellent verbal and written communications skills.
- An understanding of medical terminology

**Knowledge - Desirable**
- Knowledge of the local community and health care system and the NHS QOF and LCS’ requirements
- Good understanding of the Digital agenda within the NHS and Social Care / Care Sector

**Job Types**: Temporary contract
