We are pleased to be able advertise the new and exciting role of band 4 Community Mental and Physical Health SMI Support Worker to join our growing Older Adult Community Mental Health, Dementia and Frailty Service in Mid Essex. The Service operates across Chelmsford, Braintree and Maldon.
The SMI worker will work closely with community services and primary care colleagues, to support individuals with complex needs and severe and enduring mental illness (SMI).
The post holder will work with individuals and the qualified members of the team to identify needs and agree individualoutcomes, to create achievable care plans to achieve the individuals identified goals.
It is important that the post holder is able to work autonomously with clients managing your own caseload; adopting a flexible and positive approach whilst having the ability to deal with competing priorities. The post holder will assist in enabling patients to access the support they need to address their physical and mental health needs and promote recovery.
Role will also support comprehensive annual health checks and cardiovascular risk assessment for those with Severe and Enduring Mental Illness (SMI).
- The post holder will work as part of the multidisciplinary team.
- The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing
- To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met.
- The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests.
- The post holder will arrange physical health assessments with the service user both at home, in clinics, and at GP surgeries; taking a proactive approach to engaging patients with SMI in the importance of annual physical health reviews.
- To provide information on health promotion including healthy eating, sport and exercise and smoking cessation
- To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring the information is shared with patients and GP’s.
- To promote positive mental and physical health to patients, carers, families and staff
