Target Lung Health Check (TLHC) Co-ordinator

37.5 hours per week, Monday to Friday

Fixed term or secondment with the possibility of becoming permanent.

We are looking for a motivated and talented individual to join the Cancer Waiting Times Team at Torbay and South Devon NHS Foundation Trust. You will play a part in supporting the Multi-Disciplinary Teams (MDT) and in liaising between clinical, managerial and Admin and Clerical (A&C) staff.

This is a key role in supporting the delivery of services to cancer patients in line with national guidelines and targets.

You will learn about the role of the MDT and cover duties such as patient tracking and as needed cover the coordinating of MDT meetings.

**It would be advantageous to have**:
Good working knowledge of Microsoft Office, Trust hospital systems and experience in data management.

Experience of IHCS and Infoflex.

Knowledge of medical terminology

About your new team and department

Working on the main Torbay Hospital site and peripheral site as appropriate, within the Respiratory team.

Working within Cancer Waiting Times team of Co-ordinators, navigators and Data clerks to contribute to providing a high standard of care, which is recognised through the National Cancer Patient Experience Survey

Reducing the delay for patients referred with a suspected cancer diagnosis through to a definitive diagnosis and treatment to improve patient experience

Contribute to the service developments for continual improvements in the delivery of cancer care at Torbay and South Devon NHS Foundation Trust

Responsible for monitoring and tracking referrals arising from the TLHC following significant findings either with suspected cancer or significant incidental findings. Ensuring all relevant referrals are entered onto the Cancer Tracking Database

Support service improvement to streamline the patient journey and improve the quality of patient care.

To contribute to the achievement of the National Cancer Waiting Time target, National Audits and Cancer Registry

To support the tumour site specific Multi-disciplinary Team (MDT) in co-ordinating services to deliver the specified outcomes and targets of the Cancer Plan as required

To facilitate the patient pathway by co-ordinating and proactively tracking the timeliness of patient treatments within the National guidelines

To support and co-ordinate the Targeted Lung Health Check MDT meetings, ensuring that all the relevant people, information, notes and diagnostic results are available and discussed according to evidence-based protocols and to record outcomes and actions at the meeting

Close and effective liaison with different departments and neighbouring Trusts is an essential aspect of this role. Excellent organisational communication skills together with high levels of accuracy are therefore a key requirement for this post

To ensure all clinical and non-clinical correspondence are typed within local targets, using the Infoflex system
