Job summary

This is a new 2 year fixed post with the potential to become permanent within our Urology Cancer Service and aims to improve efficiencieswithin the pathways of care for patients diagnosed with prostate cancer.

You will play an essential part in supporting patients who have been diagnosed with prostate cancer by tracking and navigating them following their cancer diagnosis / treatment, and whilst co-ordinating their progress and liaising between clinical, managerial, nursing and A&C staff, escalating any concerns where appropriate.

The post is predominately a non-patient facing role providing direct information and support via email or telephone, and/or signposting toappropriate services for patients and their families/carers. It will also entail organising and assisting the clinical nursing team to provide the Cancer Nursing Service effectively and efficiently ensuring patients queries and care can be actioned within a timely manner.

Main duties of the job

You will liaise with the Urology Cancer Waiting Times Co-ordinator and Multi-Disciplinary Team (MDT) Co-ordinator and work closely with the Trusts' Specialist Urology Nurses to ensure patients receive timely follow up and clinical review following a prostate cancer diagnosis and treatment.

The post holder will be responsible for maintaining an electronic prostate tracker system, ensuring blood tests are requested within a specific time period, documenting on the appropriate IT systems and escalating to consultants and Clinical Nurse Specialists (CNSs) as appropriate.

About us

The Urology Team are located in Belmont Course, Torbay Hospital site but mainly at Paignton Hospital and you will be working alongside a team of clinicians, 5 cancer clinical nurse specialists, MDT Co-ordinators, and administrators. There is also a team of secretaries providing secretarial services to the whole of the urology Team.

Job responsibilities

Contributing with the day-to-day management of the PSA Tracker database within Cancer Services and liaising with various teams as appropriate across the Trust to support the service.

The post holder will be expected to work independently, but under the guidance and supervision of the Urology Clinical Nurse Specialists (CNSs) to ensure all patients on the PSA Tracker are tracked and reviewed with the clinically specified guidelines. Co-ordinating databases and both escalating and chasing any delays where appropriate.

You will provide essential and specific administration support to the team of Urology Specialist Nurses within Cancer Services at Torbay and South Devon Healthcare NHS Foundation Trust and support to the patients accessing this service.

The post is predominately a non-patient facing role providing direct information and support via email or telephone, and/or signposting to appropriate services for patients and their families/carers. It will also entail organising and assisting the clinical nursing team to provide the Cancer Nursing Service effectively and efficiently ensuring patients queries and care can be actioned within a timely manner. Processing queries and referrals, guiding and signposting clinical and patient queries.

Provide general, specific and specialist information to patients and clinical teams regarding urology cancer and cancer services, developing patient information to ensure patients are signposted at all times during their treatment and beyond.

To engage with the community services as appropriate

Assisting with the implementation of new processes and improving efficiencies.

Assisting the Urology Cancer Nursing Team with triaging telephone calls to the CNS helpline, managing emails and patient queries etc. and accurately recording patient information on the Trusts Systems

Person Specification

Qualifications

Essential

Educated to 'GCSE' level standard (5 grade C or above), or with equivalent relevant experience and skills Knowledge of specialist functional and medical terminology, AMSPAR qualification or equivalent experience to this level To be able to sit at a PC for long periods of time Undertake light lifting Knowledge and/or experience of working directly with people with cancer. Sound understanding and observance of the principles of good data management, including legal responsibilities for data protection Experience of dealing with routine and complex problems and escalation Proven ability to work to deadlines, prioritising and multi-tasking

Desirable

NVQ 3 in management or equivalent level of experience Proficient in the use of hospital computerised patient systems including Infoflex and PAS Understanding and knowledge of the National Cancer Waiting Times Targets
