JOB PURPOSE The post holder will play a key role in supporting the Personal Health Commissioning (PHC) service across the region. Working within the remit of The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC), the post holder will deliver an effective and competent level of service and consistently delivering a client-focused service. The post holder will assist and support in the facilitation of patient reviews in line with standard operating policy and procedures and ensuring The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC) is implemented and complied with, according to the principles and processes. The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

POST SPECIFIC, TASKS AND OBJECTIVES Working within a team of Personal Health Commissioning Practitioners, ensuring a high-quality service is delivered, supporting and mentoring and supporting staff to achieve continuous improvement. Managing a caseload of patients. Managing staff in line with regional and local HR policies as required. Understanding professional and personal role in relation to safeguarding processes and procedures in NHS Dorset and Local Authority Partners.

Working with health and care colleagues to ensure a quality service is delivered at all times to individuals on the PHC pathway. Assisting in co-ordinating and supporting reviews in line with standard operating procedures. Mentoring and supervision of students. Supporting the Personal Health Commissioning team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery.

Supporting the Senior Practitioner to ensure that all processes implemented are compliant with The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC) Assisting in the management of cases as directed by the Senior Practitioner and the Team Lead. Providing coordination of, and participating in, meetings and providing information, advice and support as required. Providing accurate and legible recording of information. Assisting and supporting individuals and representatives in relation to the Personal Health Commissioning process.

Ensuring Personal Health Commissioning guidance is followed and that the services are delivered to maximise patient safety. Supporting the arrangement of services required to meet the needs of individuals who qualify for NHS Continuing Healthcare and for the healthcare element of a joint care package. Supporting investigations into an individuals eligibility for NHS Continuing Healthcare following an individual challenge or dispute. Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.

Actively supporting the Personal Health Commissioning team in working towards goals and objectives. Communicating information, risks, issues and dependencies, including briefings and reports. Maintaining professional registration. Developing and maintaining knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and Children and Young Peoples Continuing Care as well as associated policies, legislation and legal judgements that impact on the delivery of the service.

Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service. Ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures. Maintaining constructive relationships with a broad range of internal and external stakeholders. Working closely with care providers involved in delivery of the service.

Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines. Fostering transparency and a person-centred approach with patients, families and representatives. Understand the importance of efficiency, effectiveness and best value in all working and commissioning practices. The post holder will be responsible for ensuring their mandatory training is up-to-date.
