We are looking for a Complex Support Worker to cover properties across Crewe. This role would involve supporting vulnerable individuals with varied needs and support levels and from a multitude of backgrounds such as domestic violence, rough sleeping and substance misuse.

The role includes:

- Providing structured support to vulnerable individuals and service users.
- Working from a trauma-informed, holistic approach.
- Providing individualised and focused support sessions, which could include anything from:
Setting up doctors’ appointments

Supporting with finance, such as setting up benefits and making budgeting plans

Coordinating with external support teams

Supporting with substance misuse programmes.
- Building person-centred support and care plans to identify the service user’s current support needs and work towards future progression.
- Identifying reasonable short and long term targets for service users to meet.
- Encouraging engagement from service users and approaching support from different angles when disengagement does occur.
- Clear, objective and professional writing up and reporting of support sessions.
- Ensuring all communication is documented and evidenced.
- Working closely with partner agencies and external support teams, fostering positive relationships with local key contacts and making referrals where necessary.
- Being capable and confident regarding lone working. Ability to recognise emotional situations and manage these accordingly. Ability to understand and work with complex and chaotic individuals. Ability to form dynamic risk assessments.
- Ideally full-time but open to part-time hours.
- Driving license required as support is spread across different areas.
- Up to £24,500 per annum depending on experience.
- DBS check required.

**Job Types**: Full-time, Part-time

Pay: £12.00-£12.50 per hour

Expected hours: 10 - 37.5 per week
