**Job summary**:
An exciting opportunity for a flow coordinator has arisen within the Cardiology team at Heartlands hospital.

The post holder will provide a comprehensive co-ordination service, working in conjunction with the multi-disciplinary teams to optimise the flow of vascular patients. The Birmingham Cardiology is based at Heartlands hospital, treating patients from across Birmingham and Solihull. The post holder will play a pivotal role in both tracking patients through their clinical pathway ensuring timely admission and discharge or transfer of care processes and pathways are being met, recording relevant data along the patient's journey.

With a focus on urgent and emergency access and inpatient stay the post holder will work closely with the clinical and operational management teams ensuring all patients referred to and cared for by the speciality are tracked have management plans that there are effective discharge or transfer plans in place. They will maintain accurate and comprehensive records of patient information which relates to admissions and discharges (planned and unplanned, including intra hospital transfers and urgent referrals and internal referrals).

They will attend handover / meetings and obtain information to enable clinical / operational teams to make informed decisions about patient flow relating to planned and unplanned referrals and discharges.

**Main duties, tasks & skills required**:
The post holder will need to be dynamic, motivated, and resourceful with the ability to work autonomously as well as being a key team member.
Have a comprehensive understanding of vascular patients including infection control procedures, surgical procedures and patient's mental and physical needs to enable decision making on discharge destinations.
Support the nurse in charge of the service/ ward / unit to monitor and ensure the most effective use of beds within the service.
Facilitate the provisions of information and co-ordinate the function of the multi-disciplinary team meetings, board and ward rounds, by ensuring lists of patients to be discussed and other relevant information are prepared
Establish effective communication networks with patients, their relatives, clinicians and other members of the multidisciplinary team, which will include electronic systems, telephone, face to face discussions and flow /capacity meetings.
Organise own day to day work and prioritise team tasks as required.
Establish robust processes with all involved in the service patient pathways to negotiate and improve the co-ordination of the patient journey
The post holder will assist in the data entry/development and maintenance of databases/ electronic patient records to capture patient information, monitoring information by proactively identifying and taking actions to enable patients to be prioritised, admitted/discharged as per individual management plan.
