**Job Description**:

- Location: Kensington
- We are searching for a Care Coordinator to join our friendly and supportive team to oversee and coordinate the care and support of the patients of the designated team. The post holder will allocate work according to demand and ensuring team members are working to their competencies.

They will ensure that KPIs are met in terms of turnaround time targets. To be proactive in recognising potential issues with the view to resolving them quickly to avoid escalation.
- The post-holder will be the go-to person for a designated team of clinicians looking after a specific patient cohort. The workload of all team members will be pooled and the Care Coordinator will have oversight of this and will assign team members to work on each stream of work according to the demand.

The Care Coordinator will sift through the workflow to ensure that it is all appropriate for the clinical team to undertake. Where any particular task is administrative, the Care Coordinator will draft this on behalf of the team. The Care Coordinator will also respond to letters from Secondary Care requesting that we carry out work that is contracted to them.

The post-holder will also attend multidisciplinary meetings/huddles on a regular basis.

The post-holder will work closely with the Social Prescribers for any social issues that patients present with, referring them to their care as appropriate.

Roles and Responsibilities:

- To attend regular multidisciplinary team meetings with and support the clinical team accordingly.
- To ensure turnaround time KPIs are met and potential problems are identified, and a resolution put in place.
- To be proactive in identifying work that can be appropriately completed by the post-holder or other non-clinical team member to support the clinical team.
- To build strong working relationships with local community services and being aware of resources available to patients with additional needs. Supporting the practice team in signposting patients to such services and resources.
- To carry out administrative duties on behalf of the clinical team, e.g. drafting TWIMC letters, completing forms, clinical coding, responding to inappropriate requests from secondary care.

To be considered for this exciting role, you must have:
- Experience of working with healthcare
- Previous experience in the NHS or social care or relevant field (desirable).
- Experience in use of GP clinical systems.
- Experience of administrative duties.
- Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality.
- Working in a multi-disciplinary setting where influence and negotiation is required.
- Knowledge/familiarity with medical terminology.
- Working in a busy and demanding environment whilst delivering in a
- timely manner.
- Vulnerable adults awareness.
- Understanding of current issues facing the NHS.
- Understanding of health and social care processes (desirable)
