Responsible to: PCN Clinical Director, PCN Clinical Supervisor and Federation Development Manager

Key Relationships: Patients Host Practice Site

Location: On site at assigned GP Practices

General Practitioners and other staff in GP surgeries PCN Primary Care team members Voluntary & community sector/groups

Hours: 37.5 hours per week

**Salary**: £27,133 per annum

Contract: Fixed term contract (minimum of 12 months)

About the Social Prescriber role

We have 2 new Social Prescriber posts available for people who are passionate about connecting people to a broad range of community groups and statutory services for practical and emotional support. Social Prescribing Link Workers will use a holistic approach to support patients to take control of their health and wellbeing based on a “what matters to me” approach and take a holistic approach to an individual’s health and wellbeing. In this role, you will support people to increase their resilience and reduce the impact of health inequalities by addressing the wider determinants of health such as debt, poor housing, isolation, weight management etc.

You will work with a diverse range of people from different cultural and social backgrounds. You should be able to establish good working relationships within the GP practices as well as within community and voluntary groups that offer activities that could be utilised for our patients.

You are passionate about working in the health and social care sector. You’re self-motived and you can prove your ability to deliver high-quality outcomes.

You will be employed by London Medical Associates, West London GP Federation, but working for one of our PCNs.

Training will be provided. A standard DBS check will be undertaken. The Social Prescribing programme is a national initiative from NHS England. We are seeking persons with a background or interest in voluntary and community sector services, health, care or public health. They will join a supportive team of Social Prescriber colleagues who work mostly hybrid. Three days in person two days remotely.

**Job Description and Personal Specification**

**Education /qualifications - Essentials**:
Qualified to NVQ Level 3 in health care or equivalent in a relevant field orHigher education qualification relevant to the role.

Evidence of continued professional development appropriate to the role.

**Knowledge and Skills - Essentials**:
Able to demonstrate an understanding of confidentiality in relation to the post, including an understanding of information governance and GDPR requirements

Knowledge of the ‘wider determinants of health’ and their impact on communities

Being able to organise and manage scheduled appointments.

Knowledge of the Voluntary and Community Sector’s role and function.

Identify and assess people’s underlying non-medical needs where they are affecting their health outcomes.

Co-produce a simple support plan identifying what services and activities the patient has chosen to take up in order to address the identified non-medical needs, and what support they require.

**Knowledge and Skills - Desirable**:
The ability to speak another community language

Knowledge of Kensington & Chelsea and Queens Park and Paddington Social and Community Assets and/or key characteristics of the strategies in place to identify and support the needs of the residents who may need Social Prescribing.

**Work Experience - Essential**:
Experience of supporting and empowering individuals to make positive changes in their lives

Experience of working in an individual support role, or experience that lends itself to this role.

Experience of working directly in a community development context, adult health and social care, public health, or health improvement.

Experience of coordinating volunteers and/or experience of being a volunteer.

**Work Experience - Desirable**:
Experience of data collection, data recording or record keeping.

**Personal Requirements - Essentials**:
Excellent communication and interpersonal skills, experienced through communicating with people at a range of levels

Ability to work flexibly and enthusiastically within a team and on own initiative.

Self-motivated, resilient and a resourceful problem solver

High levels of emotional intelligence able to quickly build rapport and positive working relationships Highly competent with electronic communications, word-processing and Excel.

**Social Prescriber Role Duties (not an exhaustive list)**

1) Coordinate the activities of local community groups, map the resources available in communities, and support building community capacity to meet identified gaps in provision.

2) Act as the lynchpin between health and care workers and local communities in order to make more effective use of social capital
