**About The Role**

**An incredibly exciting opportunity for an experienced Support Coordinator that would like to develop their career to join our team to help provide support to our residents housed in North Somerset.**

Our Support Coordinators work tirelessly and creatively with an allocated caseload of residents to provide high quality support through 1:1 and group sessions whilst working closely with external partner agencies.

We are looking to recruit a passionate and committed individual to coordinate and oversee the support delivered by our Support Workers
- To work closely with the Managers and Support Workers to co-ordinate day-to-day operational needs of the service and ensure the effective delivery of quality support services
- To be involved in the referral and assessment process for potential residents; to include booking and facilitating assessment interviews when directed, recording referral paperwork and the assessment on Saha’s support software (CPD) and notifying the applicant of the outcome of the assessment, in consultation with the Service Managers
- Support and accompany residents to attend essential appointments and signpost residents to relevant services and community-based projects;
- Actively engage clients in their support and developing and improving our services;
- Liaise with external agencies;
- Develop person-centred Support Plans and Risk Management Plans based on their residents’ identified support needs to develop the skills and resources to transform their lives and enable independent living.

**About the Service**

North Somerset Services provides 28 units of supported accommodation across 3 different sites in Weston and Clevedon. Our accommodation is for young people both single and some with children aged 16-25 years old. Residents who live at our services come from a variety of backgrounds/circumstances including relationship breakdowns, leaving care, rough sleeping or refugees. In our services we seek to provide a warm, respectful and supportive environment for clients. We want every young person who walks through the door to feel that our service is their home from Day 1. Staff aim to support and encourage the young people throughout their stay/journey with us as they move towards independent living.
