**Job title**: Case Holding Supervisor

**Location**: Stockport, Greater Manchester (Travel is involved as part of outreach)

**Hours**: 37.5 hours per week

**Salary**: £28,000 - £31,000 per annum

**The role**

Our Outreach service is a free service for families that need support navigating the complex SEND, Mental Health, Education, Housing and Social Care systems.

We have a multidisciplinary team that consists of psychologists, therapists, social workers, teachers, family wraparound co-ordinators and skilled mentors who are specifically trained to work with young people and their families to support their social and emotional skills and wellbeing.

The Case Holding Supervisor will work with and support the Operations Manager for the Outreach Division as well as the Family Wraparound Coordinators, Mentors and Volunteers.

You will help the families who are in need of practical or emotional support, particularly when difficulties arise around wellbeing and educational support. The Family Wraparound Coordinators work with the families to help them navigate the system, and link services together in a solution focussed way. You will manage them and oversee their families.

**Duties of the role**
- Conduct initial calls on referrals we receive, acting at the Great Minds Together ‘Front Door’.
- Create a child’s needs assessment and intervention plan in collaboration with the young person, family and other agencies involved if applicable.
- Provide a bespoke support and intervention plans to your families in line with their needs.
- Attend meetings and visits when required to settings such as a home or school.
- Liaise with other services and professionals about families where there are complex situations to support the volunteers & wider team
- Keep and maintain accurate records on CPOMS and other systems when required.
- Attend strategy meetings / Child in Need / Child Protection meetings and Looked After Children reviews where required
- Supervise, manage and oversee your team of Family Wraparound Coordinators. Ensuring their families are being supported, their diaries are managed and identifying training needs to promote continuous development.
- Take part and schedule regular supervisions and annual performance review meetings for your team.

**Requirements**:

- Professional experience working in the SEND and/or SEMH sector with children, young people and their families.
- Full UK driving licence with access to a vehicle (You will be required to obtain business insurance).
- Experience of managing a team of staff.
- Awareness and experience of the education system, SEND support and the social care sector including social workers, edge of care and looked after children.
- Have good interpersonal and communication skills.
- Be someone that is positive, resilient, committed and solution focused.
