**About us**

Brent K&W South Primary Care Network (PCN) is located in Brent South with 7 GP practices and a diverse population spread across Neasden, Cricklewood & Willesden.

Currently led under the Clinical Director leadership of Dr Nigel de Kare Silver, the PCN is made up of:

As a PCN we are forward thinking, innovate and driven to deliver the best patient care for our population. This includes health population management, and this role ties in with supporting that and tracking the improvements we can make to patients’ lives.

The Early Years project is a community development project run by the PCN, which promotes healthy lifestyles using local volunteers to inspire and educate others in the community.

We are looking for someone to become a Community & Health Wellbeing Champion, who will be trained and supported to work with stakeholders, using their experience, understanding and influence within their community to help friends, families, neighbours, colleagues and community members lead healthier lives.

**Job summary**
- Provide health coaching support for families with children who have one or more long-term conditions.
- Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload.
- Work as part of a multidisciplinary multi-agency team to promote health coaching.
- Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the Community & Health Wellbeing Champion role, how to refer to them.
- Support local health, social care and voluntary sector professionals to make appropriate referrals to the service.
- Promote and raise awareness of the health inequalities for children with asthma or with neurodiversity issues.
- Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
- Contribute to and take part in community workshops.
- Collect service user experience and impact of health coaching as part of the delivery of personalised care.

**Main duties of the job**
- Work sensitively with people, their families and carers to gather key information
- Work closely and establish strong working relationships with GP practices within the PCN and work collaboratively with other Health and Wellbeing Coaches, Care Coordinators and Social Prescribing Link Workers, supporting each other, respecting each other’s views and meeting regularly as a team
- Act as a champion for families with children identified of at risk of asthma and neurodiversity problems.
- Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.
- Identify opportunities and gaps in the service - and review risks and issues that could impact on service delivery - and provide feedback to continually improve the service and contribute to business planning.
- Contribute to the development of policies and plans relating to equality, diversity and health inequalities.
- Work in accordance with the practices and PCNs policies and procedures.
- Contribute to the wider aims and objectives of the PCN to improve and support primary care.

**Educational**
- Deliver personalised health promotion to families whilst providing health literacy support
- Provide lifestyle advice, healthy diet and physical exercise amongst others.
- Provide basic health education around breastfeeding and immunization.

**Clinical**
- To work with partners within the Early Years pilot site, to proactively identify children and families who would benefit from participation in the pilot.
- To discuss and identify family needs, and to gain consent for participation in the Early Years pilot work.
- To identify those eligible for childhood immunisations and other key universal health interventions such as the 2.5 year development review and encourage the uptake of missed appointment.

**Navigational**
- Support households to navigate the health and social care system and access the appropriate services for their needs.
- Proactively signpost and refer into to other existing community services, and work with families and service providers to ensure that support is accessed.

**Support**
- Develop meaningful relationships with the local community.
- Offer informal counselling and empathetic listening.
- Adopt health coaching and motivational approaches including, problem solving and goal setting.
- Undertake a “make every contact count” approach to ensure the full needs of families are identified and supported.
