Join our team as a Health Link Worker, focusing on supporting the homeless and those rough sleeping in Somerset.

As a crucial member of our collaborative team, you'll contribute to developing a service that offers early intervention and prevention nursing support, including wound care, for individuals experiencing homelessness. This includes those in hostels and supportive accommodations.

Operating within the neighbourhood and primary care directorate, your role involves providing person-centred services to individuals facing homelessness. Working in a multi-agency environment alongside colleagues in primary and secondary care, as well as voluntary agencies, you'll deliver compassionate care where needed, ensuring advocacy and empathy at all times.

Your role will address health inequalities within one of the county's most vulnerable groups. Engaging proactively with individuals in local communities, you'll contribute to a holistic approach that addresses the broader determinants of health.

This could be an excellent opportunity for a Health Care Assistant looking to take the next step in their career or for a Nursing Associate / Assistant Practitioner with a passion for working with homeless & rough sleepers.

**This role is a 12 month fixed term contract however there is the possibility it may become permanent.**

**This post is based in the Community so a valid driving licence and access to a car is essential.**

**Main duties of the job**

Supported by a Clinical Lead and a team of experienced nurses, you'll play a key role in providing appropriate and compassionate healthcare. Working closely with the clinical lead and nurses, you'll undergo necessary training to function as an autonomous practitioner and contribute to team development.

As part of a dedicated team, you'll make every contact count, directly and indirectly impacting the health and well being of individuals. Whether working under direction or autonomously as agreed with the clinical lead, you'll showcase safe decision-making and compassionate care. Your excellent communication skills will be crucial in collaborating with the service lead and nurses to support future service developments.

Expect to work with patients facing multiple complexities, such as extensive wounds, substance and alcohol use issues, acute mental health difficulties, drug-induced behaviours, and challenging behaviours. Your confidence in assessing, responding, and managing risks, with support from your team, will be essential in this role.
