**Rethink Mental Illness are hiring Support Workers to join our inspiring and friendly Community Rehabilitation Service Service team, Somerset.**

Our Open Mental Health Rehabilitation Team are based within the Somerset Community and specialise in working with people with longer-term mental health support needs. The service offers specialist rehabilitation assessment, support, and assistance to people with more complex mental health problems.

The service’s focus will be on supporting people to develop skills and confidence to live independently and to work towards each person’s own recovery goals and improved mental well-being. The service has high aspirations of recovery and well-being for everyone it supports. Our aim is to reach all of Somerset’s communities and to ensure we do this, we have developed strong links with a range of organisations across the county.

**Overview of Rehabilitation and Wellbeing Worker role**:
You will work on an agile basis in line with an agreed rota which will include working at the following venues: Somerset on site in Yeovil (The Foyer), Wells (Keward House), and Bridgwater (Willow Ward). Team office space will be provided within some locations and there is also the possibility of working some hours from home.

You will work with the following people and teams: People & OD Team, Somerset Foundation Trust, VCSE - Community Sector Enterprise alliance, Second Step, Mind, NHS teams, Willow Ward Inpatient Team, Multi-Disciplinary Team, Safeguarding, GPs, Housing Teams.

**Your role as a Rehabilitation and Wellbeing Worker will include**:

- Providing tools for wellbeing and recovery
- Psychological support
- Emotional support
- Help to find appropriate housing
- Encourage/provide links to community resources and activities
- Practical support
- Help to access education and training
- Support to find suitable volunteering and employment

**Essential experience, skills required for this role**:

- Good standard of numeracy and literacy to GCSE level or equivalent.
- Knowledge of support needs of people with moderate to severe mental health needs
- Knowledge of health and safety issues, especially those relevant to mental health
- Knowledge of safeguarding policies and procedures relating to adults and children and young people.
- Knowledge of equal opportunities issues
- A minimum of two years’ significant experience working with people with moderate to severe mental health needs
- Experience of working with people who have experienced multiple disadvantages (e.g. mental health, homelessness, alcohol/drug dependency long term conditions)
- Experience of carrying out needs and/or risk assessments and the support planning process.
- Experience of time management and competing priorities when supporting people with complex needs.
