Adults Social Worker Hospital Discharge Team

**Location**:Cheshire

**Pay rate -£37PH**

**Your key responsibilities will include**:

- The role will involve assessing people in hospital to ensure safe and timely discharge home, or an alternative care setting
- To carry out the statutory duties of the Directorate in relation to adults in accordance with Directorate policies and procedures.
- To develop skills in work within a hospital setting, including supporting discharges to the community for adults
- Completion of Strength based assessment, Mental capacity and Best interest where appropriate. Safeguarding investigations, Care planning and case management of those discharged from Hospital.

**Reed are now offering a joining bonus of up to £250 for Qualified Social Workers. Terms apply.**

**Essential Requirements**:

- SWE Registered
- Must have at least two years of post-qualifying experience working with adults in a statutory setting
- Must hold UK Driving License
