Urology Cancer Support Worker

An exciting opportunity has arisen to recruit a part time (15 hours / two days per week) Band 4 Cancer Support Worker in Urology, primarily based on the Royal Oldham Hospital site, with a requirement for ad-hoc travel to Rochdale Infirmary, Fairfield General Hospital and North Manchester General Hospital.
The cancer support worker is instrumental in support the Urology Clinical Nurse Specialists in providing exceptional patient care to patients who are progressing through various cancer diagnosis and treatment pathways, with a particular focus on supporting in the Holistic Needs Assessments of patients.
You will be supported in the role by a professional team of clinical nurse specialists, fellow cancer support workers and a lead nurse, lead urology nurse specialist and assistant director of nursing.
Provide practical support to the Macmillan Urology CNS team by contributing to patients assessment and care planning decisions through the supervision of the Macmillan Urology CNS team. To assist in the co-ordination of patient pathways for all patients who are suspected or diagnosed with a Urological cancer.
Oldham Care Organisation is part of the Northern care Alliance, one of the largest NHS Organisations in the country. Oldham is responsible for delivering a range of acute and community health services across the local area and at The Royal Oldham Hospital. As a designated major trauma site for Greater Manchester and one of only three vascular centres in Greater Manchester, Oldham is a specialist hospital for acute surgery and provides state of the art onsite pathology services to the North West
1. To demonstrate a professional approach to work and promote positive relationships with other departments / multidisciplinary teams across the Trust and Community
2. Work collaboratively with the Macmillan Urology CNS team to provide a seamless service, which is responsive to patients / carers / relative’s needs.
3. Track all biopsies taken in the Urology theatres entering them on a database, co-ordinate the necessary investigations / MDT discussion with appropriate follow up appointment based on clinical need as assessed by the Macmillan Urology CNS team.
4. To visit the Wards and review patients with a new cancer diagnosis, be able to provide empathy and support to the patient and relatives / carers whilst sign posting them to further services and onward referral to the Urology CNS as appropriate.
5. Provide basic advice to patients on the phone being able to identify specific issues and having an awareness of when the patient requires urgent treatment or to be signposted to the appropriate resource in the community.
6. Ability to promote patient centred care and establish good relationships with patient and relatives / carers.
7. Communicate effectively with members of the multi-disciplinary team from other Hospital Trusts to avoid delays in pathway and to keep patient informed at all times.
8. Assist the Macmillan Urology CNS team in carrying out Holistic Needs assessment of patients diagnosed with a Urological cancer and the implementation of their plan of care with the outcome of the assessment.
9. Design, run and evaluate Health and Wellbeing events for all patients diagnosed with a Urological cancer, to assist the Urology department in implementing the Macmillan Cancer recovery package.
10. Ability to sign post / refer patients to appropriate Community support systems as required especially when patients are diagnosed on the Wards and the CNS is unable to see the patient prior to discharge.
11. Assist the Macmillan Urology CNS team in organising Health and Wellbeing events for patients diagnosed with a Urological cancer.
12. Carry out all aspects of Nursing care as requested by the Macmillan Urology CNS team.
13. Ensure that all aspects of care / interaction with patients / carers / relatives are documented in line with Trust policy and on the departments databases.
14. Work with the Urology CNS with the management of the PSA surveillance programme, ensuring patients have their blood tests at the appropriate times by contacting them on the phone / by letter if they have missed their scheduled blood test date.
15. Triage telephone contacts providing basic advice and refer to Macmillan Urology CNS as required or signpost patients to other sources such as booking and scheduling.
16. Assist the Macmillan Urology CNS team in any audits / research being carried out within the department.
17. Assist the Macmillan Urology CNS team with any formal or informal training as required and deliver training as required.
18. Participate in meetings as requested by the Macmillan Lead Urology Nurse
19. Ensure that they are up to date with Mandatory training at all times.
