Job summary

Oldham Central Primary Care Network are looking to recruit afull time Cancer Care Coordinator to work across the PCN; the role will beutilised in the prevention, screening and early diagnoses part of the cancerpathway.

This post will particularly be supporting the early cancerdiagnosis and cancer care quality improvement work by supporting practices toimprove their processes, achieve their targets and working with other servicesto help ensure patients they have the right support at each stage of theirjourney.

Plans and progress will be supported by meetings with theGreater Manchester Cancer Alliance Early Diagnosis and Primary Care Facilitatorand PCN Cancer lead GP.

Main duties of the job

Promoting prevention through holding education events.Sourcing, creating, and distributing posters and leaflets across the PCNfootprint.

Consulting with Digital Champions within practices onadvertising preventive advice within practices social media and websites.

Working alongside local community groups to provide Patientinformation, awareness and support to enable proactive engagement/understanding.

Identify at risk populations through the QOF register.

Develop and embed systems across the network to improvecancer screening uptake, liaising with external agencies as appropriate.

Identify and contact low screening rates, non-responders andlow participation groups.

Co-ordinating referrals and manage safety netting processes,ensuring symptoms and referrals are not missed and 2-week waits (2WW) areachieved.

Gathering and interpret data to improve coding, screening,faster diagnoses, and health inequalities. Audit and Review cases wherepatients presented repeatedly before referral and late diagnoses.

Drive up standards of personalised patient care byinvolvement in the early phases after diagnosis, ensuring referral to other keyhealthcare providers such as social prescribers, health and well-being coachesand other members of the practice team, and local programmes are in place.

Support practices to deliver their quality improvement plansfor early cancer diagnosis.

Ensure patients receive a Cancer Care review in line withnational defined timescales & targets.

Experience

Essential

- Experience in a patient/customer facing role

Desirable

- Experience of working within a cancer related role or primary care.

Qualifications

Essential

- GSCE grade A - C in English and Maths

Desirable

- NVQ3 in Health & Social Care

- Access to a vehicle and ownership of a full, clean driving licence - A good standard of literacy and numeracy in line with role and duties - Exceptional interpersonal, communication and organisational skills - Excellent time management skills and ability to work in high pressure situations - Positive and confident approach to role - Display empathy and compassion towards service users and their families - Ability to adapt to shifting priorities - Personable with a positive can do, lead by example attitude - Prioritises work to maximise effective use of resources and effort - Competent administrative and IT skills - Able to use own initiative and work autonomously as well as part of a team
