The Office of Health Improvement and Disparities has awarded Salford funding to build a Rough Sleeper Drug and Alcohol (RSDA) Team. This exciting and innovative team will provide a bespoke, assertive outreach, addictions treatment model to those with substance misuse needs and are either at risk of becoming homeless or are homeless.

The team will comprise a Multi-Disciplinary Team, eg; Medics, Non-Medical Prescribers, Harm Reduction workers, Recovery Coordinators, Neuropsychologist etc and will be integrated into the Addiction Achieve model and local authority homeless services. Allocated housing outreach workers who will work alongside the RSDA Teams.

The service is aimed at supporting people facing multiple needs and exclusion leading to them having difficulty in finding and sustaining suitable accommodation and increasing their risk of recurring homelessness. The post holders will form part of an established and growing homeless team network, comprising of clinical psychology, psychiatry and dual diagnosis practitioners in different services.

As the post holder you have demonstrable experience of working in a health or social care setting and level 3 NVQ in health or equivalent experience. Knowledge of substance misuse and and its affects.

The post holder will provide community-based engagement, assessment and brief interventions to service users with substance misuse needs. The post holder will be part of the RSDA Team and will work alongside council staff, Rough Sleeper Initiative and Housing First Dual Diagnosis Practitioners, police and homeless outreach workers providing support to rough sleepers both through drop-in clinics at day centres or directly on the street whilst working alongside outreach workers.

To actively outreach to the homeless by connecting with them in their communities.

You will offer advice, information, advocacy and support to homeless substance misusers, providing a range of interventions dependent on the needs of the individual.

Within the bounds of safeguarding and confidentiality you will use a multidisciplinary assessment approach when assessing the needs of the individual, taking care to build trust to increase their engagement with services.

To case manage a small case load of identified service users with substance misuse problems, keeping them fully informed of the range of services, support available to them and the choices they have. Whilst ensuring that they are involved in developing realistic recovery plans and facilitating access to clinical, psychosocial, health, housing interventions and other services in their own communities.
