TheCare Coordinator is primarily responsible for supporting the Canterbury PCNs in the collaborative delivery of services and supporting the care for cohorts of patients. The main focus of the role will be on co-ordinating the care of identified cohorts of patients and ensuring PCN services are delivered effectively. The Care Coordinatormight be required to liaise with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional. You might also be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.
